COVID-19 screening questions

Prior to your appointment, Emma will ask you the following questions:

1. Have you or a member of your household had any of the following symptoms in the last 14 days: cough, sore throat, shortness of breath or fever for unknown reasons (e.g. not associated with this episode of breast inflammation)?

2. Have you or a member of your household returned from overseas travel in the last 14 days and are currently required to be quarantining?

3. Are you or a member of your household awaiting a COVID-19 test result or been advised to get a COVID-19 test by government officials or healthcare providers?

4. To the best of your knowledge, have you or a member of your household been in close contact with a COVID-19 known or suspected case?
